Community Care Services Program (CCSP)

The CCSP program assists individuals who are older and/or people with physical disabilities to live in their homes or with their families and/or caregivers.  It offers a significantly more affordable choice to nursing home placement by providing services in the client’s home.

Care Coordination is provided by a registered nurse and social worker to assess the client’s medical, functional and social needs.  They link clients to service providers and resources and monitor the services.

Eligibility

Community Care Services program clients must meet the same medical, functional and financial criteria as those receiving nursing home care under Medicaid.  The goal is to provide services and resources to support the client remaining in the community as long as possible.

Available Services

Care Coordination:  A registered nurse and social worker assess the client’s medical, functional and social needs.  They link consumers to service providers and resources, and monitor the services

Adult Day Health:  Provides a variety of health, therapeutic and social service activities in a group setting.

Alternative Living Services:  Provides 24-hour supervision, medically oriented personal care, routine nursing supervision and health-related support services in a residential setting other that the client’s home. This service is provided in state licensed personal care homes.

Emergency Response System:  In-Home electronic support systems for two-way communication 24 hours per day

Home Delivered Meals:  Ensures nutrition to enhance health

Home Delivered Services:  Provides home health to clients in their homes, including skilled nursing, physical, speech and occupational therapy.

Personal Support Services:  Provides a range of support services including housekeeping, running errands and basic personal care needs such as feeding, dressing, bathing and transferring.

Extended Personal Support Services:  Provides personal support services in a home setting that includes respite care for the full-time caregiver over an extended period of time.

Consumer-Directed Personal Support Services: is the same as Personal Support Services except the Consumer hires and supervises worker(s) of their choice.
